Position Summary
The Home & Community Care Nurse provides professional nursing services to clients within the community. The successful candidate will deliver client-centered care, health education, assessments, treatment, and support services to individuals and families in their homes and community settings.
Key Responsibilities
Clinical Care
- Conduct health assessments and develop care plans.
- Provide nursing care and treatment services.
- Monitor client health status and document progress.
- Administer medications and treatments as prescribed.
- Coordinate care with physicians, specialists, and health professionals.
- Provide chronic disease management and health promotion services.
- Support clients and families in maintaining independence.
Community Health
- Promote wellness and preventative healthcare.
- Participate in community health initiatives.
- Deliver health education and awareness programming.
- Assist with health screenings and clinics.
Qualifications
- Registered Nurse (RN) licensed in Manitoba.
- Current registration in good standing.
- Experience in community health or home care is considered an asset.
- Strong assessment and communication skills.
- Ability to work independently.
- Knowledge of Indigenous health issues and community-based care.
- Valid Manitoba Driver’s Licence.
- CPR certification.
Salary
Based on qualifications and experience.
